I would imagine that the reason they are holding short at 10L is that the runway entrance is via the undershoot
If you extend the runway so that you cross as per 28R i think this will solve the problem
I have just extended the runway try this

I downloaded the AFCAD again and I made the mistakes about the bad runway start locations and the runway link labled wrong I messed it up myself.

Not really a mistake but according to http://worldaerodata.com/wad.cgi?id=IT19831&sch=LICZ runway 10L/28R is only 98 feet wide, but the AFCAD has it at 150 feet.

I haven't looked at the AFCAD, but 10L/28R isn't used as an active runway in real life since it's so narrow.
